Degree Students(Bachelor / Master / Ph.D. Students)
Students admitted as degree students must complete the CCU Freshman Health Examination according to university regulations. If you attend the on-campus health examination, please bring the required documents. If you choose an off-campus examination, please submit all required documents before the deadline. |

Exchange Students(Exchange /Visiting Students)
All exchange and visiting students are required to complete the CCU Student Health Examination. The examination may be completed either in your home country or in Taiwan, provided that all required examination items meet CCU requirements. |

The Measles and Rubella Vaccination Certificate must be provided in English. If the original certificate is issued in another language, please submit an official English translation together with the original document.

Health Examination Procedure
Follow the steps below to complete your health examination successfully.

Choose Your Student StatusPlease determine whether you are a Degree Student or an Exchange Student. Different student categories require different forms and submission documents. |

Download Required FormsDownload the CCU health examination form and print it double-sided. Be sure to complete all the required information on the first page before visiting before visiting a hospital or attending the on-campus examination. |

Complete Health ExaminationAttend the CCU on-campus health examination or complete the examination at a qualified medical institution. Please ensure all examination items are completed. 📅Date:September 2, 2026 (Wednesday)

📢 Notes:
1. Fasting is not required before the health examination. However, please inform the medical staff whether you have eaten before your blood test, as food intake may affect certain test results, such as blood glucose, triglycerides, and total cholesterol.
2. In addition, please avoid wearing clothing with metal objects, such as buttons, necklaces, sequins, or other metal accessories, as these may interfere with the X-ray examination. |

You're All Set!Once your documents have been accepted, your health examination procedure is complete. Your health examination report can be collected approximately 45 days after the examination. ❓ Do I have to complete the health examination?
Yes. All newly admitted international students are required to complete the health examination according to CCU regulations.
❓ Can I complete the health examination in my home country?
Yes. Students may complete the examination in their home country, provided that all required examination items meet CCU requirements.
❓ What if my medical report is not written in English or Chinese?
Please provide an official English or Chinese translation when submitting your report.
❓ What happens if I miss the submission deadline?
Late submission may affect the completion of your student health record and other related administrative procedures. Please submit all required documents before the deadline.
❓ How much is the on-campus health examination fee?
The on-campus health examination fee is NT$680. Please prepare cash.
❓ Can I visit any hospital for the health examination?
Yes. You may visit any qualified hospital. Please make sure that every required examination item listed by CCU is completed.

If any abnormal findings are identified during your health examination, please follow up according to the abnormal items and complete the required re-examination.
Please submit the re-examination form to the Health Section's case manager by the required deadline.
