1️⃣本校體檢表：請至下載今年最新版本，以A4尺寸紙張正反面列印成1張，自行填妥第一頁內容

2️⃣體檢費用：新臺幣680元

3️⃣請搭乘電梯上樓，以免影響血壓數值；胸部X光檢查車停在管理學院門口

4️⃣8/31-9/2體檢會場撿到雨傘2支及水壺，請到衛保組櫃台領回~

(Above the Office of Academic Affairs)

🔴 What to Bring / Prepare:

1️⃣ CCU Health Examination Form
Please download the latest version of the health examination form for this academic year. Print it double-sided on one A4 sheet of paper, and complete all the required information on the first page before coming to the examination.

2️⃣ Health Examination Fee:
NT$680

3️⃣ Transportation to the Examination Venue
Please take the elevator to the 3rd floor to avoid affecting your blood pressure measurement.
The mobile chest X-ray unit will be parked at the entrance of the College of Management.

4️⃣ Lost and Found
Two umbrellas and one water bottle were found at the health examination venue during the August 31–September 2 health examinations.
If these items belong to you, please visit the Health Center counter to claim them.
