(一)健康檢查時間：

1. 日期：115 年 3 月 5 日 星期四

2. 時間：9：30~13：30（請於13：10前報到）

3. 地點：行政大樓東棟3樓宴會廳

4. 體檢費用：630元

5. 檢查對象：提早入學生、轉學生及復學生

6. 檢查時間約30分鐘，依當天情況為主，建議預留時間。

(二)準備用物：

本校體檢表及體檢費用新臺幣630元。

體檢表請以A4尺寸紙張雙面列印，第1頁為自填部分，請填妥所有資料帶至現場。

交換學生須增加短期研修健康檢查表（丙表）體檢項目。請自備MMR接種證明影本(英文版本)，於現場查驗及繳交。

(三)體檢須知：

1. 健康檢查前3天請維持正常作息、勿暴飲暴食。體檢當日，請依您自身的身體情形評估是否空腹體檢，只需於抽血時告知檢驗人員是否進食即可。(進食可能影響之檢查項目：血糖、膽固醇、三酸甘油脂）。

2. 體檢當日，請儘量不要配戴隱形眼鏡，以利檢查裸視。若有必要佩戴隱形眼鏡，請記得自行攜帶隱形眼鏡保存盒。

3. 請避免穿著有金屬物件（鈕扣、項鍊、亮片等）之上衣，以便檢驗胸部X光；孕婦或可能懷孕者請於胸部X光檢查前告知醫護人員。

The examination will take approximately 30 minutes. The actual duration may vary depending on the situation on that day. Please allow sufficient time in your schedule.

Page 1 is the self-completed section. Kindly complete all required information and bring the form with you on the examination day.

Exchange students are required to complete the additional health examination items listed in the Short-Term Study Health Examination Form (Form C).

Please bring a photocopy of your MMR vaccination certificate (English version) for on-site verification and submission.

(3) Important Notes

• Please maintain a regular routine and avoid overeating or binge eating for three days before the examination.

• On the day of the examination, assess your own physical condition to decide whether to fast. You only need to inform the medical staff at the time of blood collection whether you have eaten.
  (Eating may affect the following test results: blood glucose, cholesterol, and triglycerides.)

• Please avoid wearing contact lenses if possible to facilitate the unaided vision test. If you must wear contact lenses, please bring your own contact lens case.

• Please avoid wearing tops with metal objects (such as buttons, necklaces, sequins, etc.) to facilitate the chest X-ray examination.

• If you are pregnant or suspect you may be pregnant, please inform the medical staff before undergoing the chest X-ray examination.
